Prenatal Diagnosis of a True Knot of the Umbilical Cord in Twins Pregnancy

چکیده
Background: True knot of the umbilical cord is an uncommon finding, which is difficult to diagnose antenatally. The appropriate management of patients with a sonographically identified true knot is yet to be determined. Case: We present a twin pregnancy, in which antenatal diagnosis of a true knot of the umbilical cord was made at 33 weeks’ gestation. Fetal Doppler examination showed an absence of end diastolic flow in the umbilical artery of the second twin. Conclusion: The appropriate management of patients with a true knot identified prenatally is unclear. Once diagnosis is suspected, further fetal surveillance is warranted. Figure 1: Longitudinal sonogram of the abnormal portion of the umbilical cord demonstrates a bulge due to a loop of blood vessels. Scans show exaggerated looping of vessels. cord insertion of the second twin was revealed (Figure 1). On color Doppler this finding demonstrated a circular like pattern. Proximal and distal to this finding, the umbilical cord looked regular in appearance. Pulsed-wave Doppler examination revealed an intermittent absence end diastolic flow pattern in the umbilical artery of the second twin. Because of the early gestational age and the evidence of fetal wellbeing, expectant management was chosen, and the patient was admitted

Background: True umbilical cord knot is one of the abnormalities of the umbilical cord. Active fetal movements create cord knotting. True umbilical cord knots are rare but may be associated with fetal distress and stillbirth. True umbilical cord knots are capable of impeding blood flow to the fetus.Case presentation: A 26-year old primigravid woman was first treated for genital herpes simplex v...

Introduction: True knot of the umbilical cord is characterized by a cord loop with increased tension and hence higher risk for obstetric and neonatal outcome intervention. Original Research Article British Journal of Medicine & Medical Research, 4(31): 5043-5052, 2014 5044 It is a finding observed in 1-2% of births. Aim: Review the etiology, pathophysiology, diagnosis and prognosis of true knot...
متن کاملPrenatal ultrasound diagnosis and pregnancy outcome of umbilical cord knot – debate regarding ethical aspects of a series of cases
True umbilical cord knot appears to be a relatively common complication that occurs in 0.3%-1.3% of all pregnancies and it is correlated with an increased incidence of SGA infants, premature birth, need for neonatal intensive care and fetal death. The aim of the article was to evaluate the incidence of the true umbilical cord knot in the University Emergency Hospital, Bucharest, for a period of...
متن کاملTrue Umbilical Cord Knot Leading to Fetal Demise
The incidence of true knot of the umbilical cord is not only very low but it is often undiagnosed antenatally when present despite the availability of prenatal ultrasonography. When the true knot remains tight, it may impede the circulation of the fetus and may result to fetal death in utero especially in labor. We report a very uncommon case of true knot of the umbilical cord that ultimately l...
متن کاملI-40: Ultrasound in Multiple Pregnancy
Ultrasound has revolutioned the care of women carring twins. First trimester evaluation is the best time to determine chorioniocity and amnioniocity in multiple gestations. First trimester diagnosis is based on the number of gestational sacs, amnions and yolk sacs. Growth rate in multiple multiple gestations during the fist and early second trimesters parallels the growth rate of singleton preg...
